Scoring between 95% and 100%, a probation officer marked Basil Borutski’s performance as meeting the target just two days after he killed two former partners, Anastasia Kuzyk and Nathalie Warmerdam, for whom he was on probation due to abuse. Borutski also murdered Carol Culleton on September 22, 2015.
In the years leading up to the tragic events in the Ottawa Valley, Borutski’s probation officers were responsible for ensuring that the repeat domestic violence offender followed court orders after serving jail time for offenses against Warmerdam’s family and assaulting Kuzyk.
Despite efforts to coordinate with various agencies to ensure the safety of the victims, including Kuzyk, Warmerdam, and Borutski’s ex-wife, the local manager’s focus, shortly after the murders, was on the lack of monthly contact with Kuzyk.
Following a death review, it was revealed that multiple chances for probation officers to intervene had been overlooked, indicating significant missed opportunities.
Expressing disbelief, Warmerdam’s son and a lawyer representing victim advocacy groups during an inquest into the murders criticized the probation system for its handling of Borutski’s case.
Management Reviews Conducted Before Tragedy
Prior to the murders, Borutski’s case was reviewed twice by the same area manager, revealing instances where Borutski failed to comply with probation conditions related to attending domestic violence offender programs.
Despite warnings and concerns raised by various parties about Borutski’s behavior and potential risks, the probation officers’ approach did not show significant changes leading up to the tragic events.

Although updates have been made to the management review tool, concerns persist about the effectiveness of probation oversight, prompting calls for systemic improvements to better protect victims of intimate partner violence.
